How to file for, Aetna Supplemental Sickness Benefits

1. The notice of claim must be given within 20 days of the start disability or as soon as reasonably possible. If the employee does not submit his notice of claim within the prescribed 20 day period his/her claim will be denied unless they provide Broadspire with sufficient information to establish, as determined by Broadspire, that although he/she did not meet the 20 day time limit, he/she did submit their notice of claim as soon as reasonably possible.

2.The form to be used for the submission of a claim appears in the booklet between pages 8 and 9.
